How We Operate

Home Is Where The Heart Is Home Care provides the best personalized in-home care. The agency started 10 years ago in Steubenville, Ohio, and has grown to service the Ohio Valley, Northeastern Ohio, and the Northern Panhandle of West Virginia. The agency strives to hire the most knowledgeable, compassionate State Test Nurse Aides, Certified Nurse Aides, Patient Care Techs, and other employees with relevant facility experience. Using a software system that is downloaded onto each caregivers' phones allows management to know the caregiver's location due to Geofencing on the app. Our software will alert management if a caregiver is late clocking in. This provides peace of mind for our families and their loved ones.

Home Is Where The Heart Is Home Care works with the family and loved ones from the initial phone call. Our client caregiver liaison will ask you to explain your loved one's situation, and we will explain to you how we will benefit your loved one. We work with the family to develop an individualized care plan. This care plan will be viewed by the caregivers on their phones. They will see all information regarding the client along with each task they will be doing daily. This information is gathered from the new client packet with a care plan. The app will allow the caregiver to mark off the tasks as they are completed and make all care notes on the app. At the end of the shift, the caregiver will clock out and ask the client to sign their name along with the caregiver signing their name on the app, indicating everything was completed to their satisfaction. The caregiver will then clock out, and this will end their shift.

Our Client/Caregiver Liaison will work with the family to get the New Client Packet with the Care Plan completed and will work to make sure when your loved one comes on with our agency that all information is inputted, the care plan is set, the Family Room Portal will be set up when our Client/Caregiver Liaison is doing your family orientation with our agency. The Family Room Portal is an integral part of the agency. You will be able to view caregiver care notes, medications, invoices, and more.
